Guidance & education for those supporting a loved one who is suffering from substance-use addiction. Participants in the Recovery Support Navigator program are able to access the therapeutic counseling and psychiatric services of Liberty Resources Behavioral Health Center, located in the same operational space.Engages individuals and their loved ones in recovery-focused activities including substance–use-disorder and opioid-abuse treatment, counseling, support groups, peer interactions, and community services.Ĭonnecting with Integrated Health Services.Help in understanding the progression of substance-use-disorders and navigation of the treatment service-delivery system.Peer Advocates provide emotional support, share insights about the treatment process, and assist the client in obtaining services.The team is composed of both Family Support Navigators and Peer Advocates that are community based and embedded in hospital and detox settings.Critical connections for a person’s start on the path to recovery.The Peer Advocate helps establish connections to addiction treatment after an opioid-overdose reversal, after discharge from another addiction-related ED visit, or as needed in other settings.The FSN will develop relationships with Monroe County addiction prevention, treatment and recovery services, managed-care organizations, local substance-use-disorder councils, and community stakeholders too. The Family Support Navigator (FSN) assists families with accessing local treatment and support service. The Recovery Support Navigator Team provides family-focused (or other loved one–focused) services designed to guide, educate, and support families through the substance-use-disorder treatment and recovery process through peer and family support. Liberty Resources is accepting new patients ages 18 and up. Liberty Resources Primary Care has been awarded Patient-Centered Medical Home (PCMH) recognition by the National Committee for Quality Assurance (NCQA) for its evidence-based, patient-centered processes that focus on highly coordinated care and long‐term, participative relationships.
